Though not necessary, it is preferred to fill in the forms before your arrival.  This provide us a little more information about yourself so we can get prepare and also makes the intake process a little faster.

Those prefer to do it on paper, please print and complete the following forms and bring them with you to your first visit.  You only need to fill these forms once.
